Description

Assist referral sources in identifying patients with care needs that match services provided. Educates potential patient and family regarding the services provided, facilitates the admissions process, communicates with appropriate referral source staff and ensure s accommodations and staff are prepared for the patient.

  • Provideon-site clinical assessment and assist referral sources with identification ofappropriate patients including education regarding admissions criteria

  • Communicatetransfer, admission, clinical and financial information to appropriate facilitystaff members.Ensure that the facilityand the agency are prepared for the admission

  • Meetwith patients and families to discuss the transfer process, anticipated treatment,expected outcome and financial implications

  • Obtainnecessary medical and financial information relevant to the completion of theadmissions process

  • Managethe relationship with referral sources and serve asthe primary source for information to the referralsources regarding current services

  • Conductregular visits to assigned hospitals and facilities to build and maintainrelationships with Social Workers, Discharge Planners, Case Managers andPhysicians to ensure that the agency is responding promptly to meet patientcare needs.Conduct regular visits thatserve as educational opportunities for the referring facility staff

  • Complete all documentation electronically as required

Requirements

  • Must adhere to the Company’s Code of Conduct and Business Ethics policy including documentation and reporting responsibilities

  • Registered Nurse or Licensed PracticalNurse in good standing currently licensed by the State in which the servicesare provided

  • Musthave current certification for CPR

  • Mustbe able to relate positively and favorably to residents and families and towork cooperatively with other employees at all levels

  • Abilityto travel extensively, work on-site in numerous facilities and work eveningsand weekends when needed

  • Must be capable of maintaining regular attendance

  • Basic computer skills to include electronicmail and Microsoft Office

  • Knowledgeof third party reimbursement

  • Selfdirected and able to work with minimal supervision

  • Excellentverbal and written communication skills

  • Mustbe able to maintain confidentiality regarding patient and company proprietaryinformation
